At the point of referral, I will ask for some background information on your condition and your occupational therapy goals to inform my assessment.

I will send you a link to the referral form and consent form by email.

Paper or telephone referrals can be accommodated as required.

Your initial assessment can take place in whichever environment is most appropriate for you and your therapy needs, for example in your own home, at work or in the community. I also cover residential and nursing homes.

I will consider your physical, cognitive and social needs, as occupational therapy assesses the whole person.

From my assessment, I will recommend appropriate interventions, with clear clinical reasoning.

This may be a standalone solution, such as equipment provision, or a number of rehabilitation treatment sessions. Goals and progress will be reviewed regularly as part of treatment sessions.
